Surgery Preparation Floyd Outpatient Surgery Center, 330 Physician Center
The Floyd Preoperative Evaluation Center offers assistance in preparing you for surgery. Once your surgery is scheduled, a registered nurse from our Preoperative Evaluation Center will call you to ask some general health questions and determine if an appointment for preoperative evaluation is necessary.

Instructions for the day of your surgery, including what to do with your medications
Discussion of the procedure and aftercare
Time for you to ask questions
Items needed for your preoperative evaluation appointment include the following:
Physician's order and consent forms for your procedure, if not already provided.
Proof of insurance
Photo identification
Wallet card for heart stents or pacemakers
All your medications in a bag
